$3,000 Sign-on Bonus Eliot is seeking a Masters Level candidate preferably in rehabilitation counseling as a MA Vocational Case Manager for the Program for Assertive Community Treatment (PACT) team in Lynn. Our ideal candidate will have strong clinical skills and experience with job development, job coaching and skills assessment. PACT is an evidenced based multidisciplinary outreach mental program that serves adults with serious and persistent mental illnesses in the community. PACT clinicians work as a team to collaborate with consumers to develop treatment interventions that assist them in their recovery process. Responsibilities: Act as a primary case manager to 7-15 consumers in accordance with Eliot's policies and PACT standards Participate in treatment planning meetings on a weekly basis through written documentation and clinical discussions. Provide individualized vocational support to assist consumers in identifying their strengths, barriers to work, establishing goals and ways to manage symptoms and work. Take the lead role in providing vocational services to consumers on the team. Participate in Daily Organizational Meeting (DOM) in accordance with Eliot policies and PACT standards. Develop sound and working relationships with local employers, career centers, employment advocates and DMH in order to assist consumers in finding employment Providing job coaching to individuals employed competitively in the community. Accompany individuals to the workplace, observe their performance, and offer assistance and support as needed. Interface with the client's supervisors to ensure that the individual is completing the designated tasks as assigned. Schedule: Monday - Friday 8am to 4pm, with one 12pm-8pm shift per week and one weekend per month. Salary: $61,276.80 annually 6% bilingual salary increase if fluent in the needed language capacity The sign-on bonus is prorated for positions under 40 hours per week Eliot is committed to identifying and dismantling barriers that prevent people with marginalized racial identities from actualizing their full employment potential, assuming leadership roles, or from fully engaging at all levels in the workplace.
