PACE - Essential Duties and Responsibilities:

The RN will be required to provide care to the PACE participant mainly in home, but not limited to the clinic or day center setting.

Complete initial and annual levels of care

Complete and record vital signs.

Observe participants under treatment to identify progress, side-effects of medications and change in condition.

Communicates treatments and self-care interventions with participants and their families/caregivers.

Administers prescribed medications and injections

Provides wound care and assessments.

Inserts and monitors urinary catheters per provider orders.

Will accurately document within the electronic medical record (EMR) all clinical documentation including but not limited to:

o Assessments,

o Progress of care,

o Vitals, and

o Communications with participants

Educate participants and caregivers on ways to keep out of the hospital, SNF, and/or LTC.

Will provide in home assessment for post hospitalization and falls and will document clinical findings in the EMR.

Will provide medication education to participants and caregivers per provider orders.

Participate in the Clinical On-Call services at PACE Your LIFE.

Work collaboratively with the Occupational and Physical Therapist on in-home services needed.

Reports directly to the Home Care Coordinator

Knowledge and Skills:

Comfortability with providing care to participants within their home

Knowledge of medical terminology

Ability to undertake a variety of diverse care tasks and work in an interdisciplinary team

Compliance with all HIPAA policies and regulations

Compassionate and able to handle stress

Qualifications:

1. Education:

a. Graduate of an accredited school of professional nursing

2. Credentials/Licensure Required:

a. Current active license as a Registered Nurse in Delaware

3. Experience:

a. Two years professional nursing experience required. Long-term care and/or home health care experience preferred

b. One year experience working with elderly individuals preferred, but must all other regulatory requirements.

c. Familiarity with State and Federal regulatory requirements preferred

d. Proficient use of Electronic Health Records, Outlook, Word, and Excel preferred

4. Age of Patients Rendered Care:

a. Adult and geriatric patients

Freedom from illegal use of drugs, and freedom from use and effects of use of drugs and alcohol in the workplace.

Persons who have been found guilty by a court of law of abusing, neglecting or mistreating individuals in a healthcare-related setting are ineligible for employment in this position.
