For over 75 years, Catholic Family and Community Services, has provided at little or no cost, a full spectrum of services to individuals who are oppressed, families who are in crisis and communities who are disenfranchised. Located throughout the New Jersey Counties of Passaic, Morris and Sussex, we provide professionally based social services, including Behavioral Health Programs, Veterans Services, Childrens Programs, Senior and Aging Services and much more. Outreach Worker SSVF Program Schedule per week: Monday - Friday 8:30am - 4:30pm Location: Paterson, NJ SSVF is a homeless prevention program that assists low income veteran families in maintaining their housing and rapidly re-housing homeless veterans and their families. The Outreach Worker participates in outreach to identify homeless veterans in the Morris, Sussex, and Passaic counties and to present the SSVF program as an alternative. PRINCIPAL DUTIES AND RESPONSIBILITIES: 1. Network with Morris, Passaic and Sussex County providers, parishes, community government organizations to effect maximum utilization of available resources to meet client needs. 2. Manage time and work independently with accuracy and complete adherence to policy and procedures. 3. Provide referral services to help meet additional needs. Assess eligibility for community resources. This will include homeless families and families in financial crisis. 4. Assess program qualification criteria of client and aid in qualifying or referring to outside resources. 5. Provide ongoing support, crisis intervention and housing stability support and resource referrals. 6. Keep outreach log of all outreach attempts and community resources obtained 7. Conduct home visits to support client in housing stabilization. 8. Participate in street outreach to identify homeless veterans and present the program as an alternative to homelessness. 9. Liaison with the VA outreach departments, community outreach departments, VASH department and the VA benefit and hospital departments. 10. Visit jails and prisons to present program to inmates and staff. 11. Participate in conferences, workshops, special projects, staff meetings and other duties as assigned. 12. Valid Drivers License in good standing Knowledge of New Jersey housing programs, laws, housing codes, and community resources Ability to work with a cross section of the population including homeless individuals and families Ability to work independently and work with other staff to ensure appropriate levels of treatment/support Strong organizational skills and excellent communication skills (writing and speaking). Working knowledge of computers and MS programs. Ability to serve as a liaison between consumer and third parties.
