POSITION: Outreach Specialist
REPORTS TO: Director of Growth
FLSA STATUS: o exempt
POSITION SUMMARY: Promote awareness and education of Hospice and Palliative Care services to the health care community and act as liaison between organization and providers
EDUCATIONAL QUALIFICAITONS:
- Bachelor's degree in marketing, human resources, nursing or related field preferred.
- Minimum 2-3 years' professional experience required
- At least 2 years' experience in end-of-life care preferred
- Hospice experience strongly preferred
- Demonstrated knowledge of hospice and palliative care
KNOWLEDGE AND SKILLS:
- Knowledge of and commitment to the hospice philosophy of care
- Ability to work on an interdisciplinary team
- Ability to work a flexible schedule
- Willingness to work with patients/ families regardless of race, color, creed, age or sexual orientation. Must respect their beliefs and philosophy with an attitude of openness and receptiveness
- Excellent communication and interpersonal skills
Requirements:
TRAINING-Initial And Ongoing:
- Complete Hospice volunteer/staff training program
- Complete Hospice orientation
- Obtain eight hours of continuing education per year, including mandatory safety in-service
- Attend minimum of six staff meetings per year
DUTIES AND RESPONSIBILITIES:
- Assist in orienting new team members, as requested
- Participate in quality improvement program
- Adhere to strict confidentiality
- Adhere to safety policy and practices
- Complete employee time record accurately on a timely basis
- Cooperate with all government agencies and outside auditors
- Assist with updating policies and procedures, as appropriate
- Complete annual TB screening
- Assure all license, certifications, continuing education information, and any other required documentation are submitted to human resources and kept current
- Maintain up-to-date documentation required and specified in Hospice policy and procedure
- Perform other duties assigned by supervisor
- Demonstrate knowledge of federal, state and other regulations governing hospice care
- Work with supervisor to design and carry out a strategic marketing plan for health care providers
- Make frequent and ongoing contacts with all area hospitals, adult care homes, skilled nursing facilities, home health providers and key physicians based on strategic plan
- Develop, maintain and analyze on an on-going basis data on all providers
- Provide regular and informative reports on all activities and future strategies
- Work closely with care teams to maintain knowledge of special needs or concerns
- Educate providers about referral process and assist as needed
- Coordinate ongoing education for all new and current physicians in the service area making regular visits to key physician offices
- Address referral partner complaints or concerns, collaborate with team and follow-up with customer to ensure satisfaction
- Maintain brochures and appropriate written materials in health care provider locations
- Demonstrate understanding of nursing home regulations
- Work with interdisciplinary team members to establish and maintain positive working relationships with referral partners
- Develop educational materials and programs for facility personnel
- Cultivate relationship with hospitals across geographical service area
- Cultivate relationships with home health and related providers through regular and ongoing contacts
- Identify and coordinate education programs for providers
- Assist public relations and marketing coordinator with coordinating and implementing special days such as doctor's day, nurse's week, etc.
- Serve as Hospice representative on appropriate community initiatives
PHYSICAL QUALIFICATIONS:
- Speech and hearing skills necessary for telephone interaction and face to face contact
- Visual acuity to read chart and physician's orders
- Ability to stand and walk for long periods of time
- Manual dexterity with the ability to manipulate office equipment and keyboard for an extended period of time
- With assistance, ability to lift and support an average of 30 lbs
- Ability to sit as required to perform duties
- Ability to pull/push, and bend frequently
- Ability to squat, kneel, climb, and balance
SPECIAL REQUIREMENTS:
- Must have an automobile with required insurance coverage and state driver's license
