Outpatient Social Worker

Job Summary: In this position, the Social Worker will provide comprehensive psycho-social assessment, management, and support to at-risk patients and families in the Family Medicine and Internal Medicine outpatient clinics. In addition, the social worker addresses social determinants of health and complex issues, assisting and collaborating to make referrals with multiple community agencies to meet the needs of the patients and families. This position is part of the interdisciplinary team that recommends and delivers appropriate social work intervention and support. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities. Minimum Qualifications: M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Must obtain L.M.S.W. within 12 months of receipt of limited permit. Preferred Qualifications: Work Days: Monday- Friday, Day Occasional on-call/holiday coverage on rotation based on operational needs.
