Outpatient Care Coordinator Registered Nurse

Qualifications

- Registered Nurse
- Graduation from an accredited school of nursing.
- Current registration with the Department of Professional Regulations in the State of Illinois.
- Previous experience in caring for chronic disease patients is required.
- Minimum of 5 years of experience in a healthcare setting, preferably primary care.
- Previous experience with mobilizing community resources, and navigating through the healthcare continuum.

- Coordinates team-based care within the Accountable Care Organization (ACO), including hospital, physician, and other providers
- Monitors plan to provide exceptional and efficient patient care.
- Facilitates a shared goal model within and across settings to achieve coordinated care that is patient/family centered.
- The position is key to bridging gaps that occur in care transition.

Schedule

- Full Time
- Monday-Friday
- 8:00 am - 4:30 pm

Job Type: Full-time

Pay: From $26.00 per hour
