locations Great Bend, KS time type Full time posted on Posted Yesterday job requisition id R-33764 Position Title Orthopedic Nurse Navigator Great Bend Hospital Position Summary / Career Interest: Under the supervision of the nursing department's management, the RN assumes the responsibility for the nursing processes of assessment, diagnosis, outcomes identification, planning, implementation, and evaluation needed to deliver safe, evidence based care within their scope of practice. Responsibilities: Collects and assesses initial and on-going data about the health status of the patient. Initiates and/or revises an individualized plan of care that includes immediate and long-term outcomes for the patient/family and reflects the plan of the entire health care team. Carries out interventions safely and in compliance with policy to facilitate achievement of expected outcomes. Evaluates patient response to intervention/therapy based upon expected outcomes. Provides patient education by explaining procedures, medications and giving instructions at a level the patient can understand; addresses patient's questions and concerns; verifies patient informed consent when indicated. Documents and communicates all required components of patient care. Provides direct patient care in the inpatient perioperative setting. Documents and communicates all required components of patient care on chart forms, birth certificates, and computer. Initiates fetal heart monitor, performs vaginal exams, interprets fetal heart tones and determines frequency duration and intensity of contractions. Monitors progress of laboring mothers, interprets fetal heart monitor and notifies physician when appropriate for progress or problems. Assists mother and labor coach with breathing exercises, pain management techniques and administers pain medication as ordered. Prepares birthing room for delivery using strict sterile technique. Prepares mothers for elective or emergency Cesarean Section by following written C-Section protocol. Provides emotional support to mother and coach, obtains proper consents and pre-op diagnostic studies and coordinates smooth transfer to surgery. Provides patient education by explaining procedures, medications and giving instructions at a level the patient can understand; addresses patient's questions and concerns; verifies patient informed consent when indicated. Documents and communicates all required components of patient care. Works collaboratively with the physician and the anesthesia health care providers to determine patient needs undergoing surgical procedures and to assume responsibility for patient care, document and evaluate the effectiveness of nursing actions and ensure a safe environment of care. JOB REQUIREMENTS Required: Requirements GB Pain Clinic Graduate of an accredited School of Nursing. Current state nursing licenses or RN license from a participating state in the National Licensure Compact. BLS PALS ACLS Requirements GB Labor and Delivery Graduate of an accredited school of nursing. Knowledge of general nursing theory and practice required. Knowledge of principles, methods and techniques involved in performing general nursing services and adapting or modifying standards of nursing practices for care of specific medical cases. Current RN licensure in the State of Kansas or has work permit from the State Board of Nursing. ACLS Neonatal Resuscitation (NRP) within six months. Fetal Heart Monitoring (FHM) within one year of employment PALS within 6 months of employment. BLS certification with in the first two weeks of employment. Requirements GB Inpatient Nursing Graduate of an accredited school of nursing. Prior experience in specialty field preferred. Knowledge of general nursing theory and practice. Current Registered Nurse (RN) licensure in the State of Kansas or has work permit from the State Board of Nursing BLS certification with in the first two weeks of employment. ACLS certification required within 6 months of employment. Requirements at GB Emergency Department Current Kansas Registered Nurse License Graduate of an accredited school of nursing Bachelors of Science in Nursing (BSN) required or must have plan to obtain BSN approved; in some cases, approval of experience or certifications may substitute for BSN. Knowledge of general nursing and practice. Cardiac Rhythms class required or must complete within six months of employment. Current Registered Nurse (RN) licensure in the State of Kansas or work permit from the State Board of Nursing. ACLS certification within 90 days of employment. BLS certification with in the first two weeks of employment. Knowledge of principles, methods and techniques involved in performing general nursing services and adapting or modifying standards of nursing practices for care of specific medical cases. TNCC and ENPC required within one year of employment. Requirements GB Pre-OP/PACU Graduate of an accredited School of Nursing required for new graduates, preferred for experienced RN's. One year experience preferred Nursing skills and knowledge base specific to specialty Current state nursing licenses or RN license from a participating state in the National Licensure Compact BCLS certification PALS Certification required within 6 months of employment. ACLS Certification required within 6 months of employment. Preferred: Preferred GB Pain Clinic One year experience preferred Nursing skills and knowledge base specific to specialty. Preferred GB Emergency Department Certified emergency nurse preferred. Preferred GB Pre-OP/PACU Certified OR Nurse (CNOR) HIPAA: This position will have access to the following Protected Health Information in order to carry out the duties related to their position based on the following criteria: Primary - required (routine) to do the job Secondary - required for the job, but mostly be exception None - no approved access Description of Information Level Primary: Patient Demographic Information (information used to identify a person): Name, Date of Birth, Address, Race, Marital Status, Religion Financial Information/Insurance (information related to insurance, billing and payment): Billing Information, Payer Name, Payer ID, Account Balances, Plan Elements Covered, Payment Information, Payment Rates Clinical Information (information that describes a patient's health status): Diagnosis, Reports/Medical Notes, Test Results, Problem List, Procedures, History and Physical Coding Information (clinical information that is in (alpha) numeric format): ICD-9 Codes, Rev Codes, CPT Codes Infection Control: Initial and Ongoing training in dealing with infection control. Training's could include but are not limited to, blood borne pathogens, bodily fluids and bio hazardous materials as it applies to your daily work environment.
