The RN would be on call from Monday through Thursday, The RN would be responsible for covering any as-needed visits that arise. Compensation will include an on-call stipend PLUS pay per visit for each visit completed. The Nurse will assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. May advise patients on health maintenance and disease prevention or provide case management. A licensed Registered Nurse is required.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • On call for Weekdays (Monday - Thursday).
  • Responsible for providing on-call patient visits scheduled, or unscheduled, for urgent physical and psycho-social issues during after-hours scheduled shifts. This may include admission visits.
  • Available via cell phone to respond to visit requests and communicates effectively with the Hospice team, including the Hospice physician, to address urgent patient needs and establish a safe plan of care.
  • Implements the plan of care, educating patient/primary caregiver on changes to the plan of care, including medication management, patient safety, and symptom management.
  • Provide skilled nursing treatments per plan of care.
  • Coordinate with physicians, therapists, and other individuals in the patient's care plan
  • Attends patient deaths as directed to provide emotional support and guidance to the family.
  • Maintain complete and accurate, and timely documentation of nursing care
  • Work collaboratively with all members of the hospice team
  • Reports off to the primary Hospice team concerning patient-related visit, changes to care plan, and follow-up needs.
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)

  • Associates degree required; Graduate of an accredited state approved, Bachelor's Degree from school of nursing preferred
  • Current and valid Virginia Registered Nurse License
  • Prior nursing experience, specifically in hospice (preferred)
  • Current CPR certification (required)
  • Driver's License, Car Insurance, Car Registration
  • Strong interpersonal skills that build personal rapport and establish trust with the client
  • Computer literate
  • Strong organizational skills
  • Excellent written and communication skills
  • Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)
  • Home health care experience
  • Bilingual (Spanish) preferred
