The schedule for this position is on call - per diem. There are three residential treatment homes within Clackamas County, in Oregon City or Gladstone. A shift differential may also be included, depending on the shift you work. Both Swing and overnight shifts are provided with a shift pay differential in addition to the hourly wage when clocking in; swing is .50 cents and overnight is $1.00. Choice Model programs have a goal to divert individuals from Oregon State Hospital (OSH); coordinate successful discharge from OSH into appropriate community placements; coordinate care for individuals residing primarily in licensed residential facilities in order to move individuals into the least restrictive housing possible; and care coordinates and develops supports to Mental Health Aide I or II work as a member of an interdisciplinary team to implement and monitor treatment for residents within the residential facility. This position involves participation and implementation of program goals and objectives to promote a strengths-based, client centered approach to assisting residents in achieving individual goals and ultimately transitioning to a less restrictive setting. The work requires one to stay awake and aware on all shifts. Shifts are day, swing and overnight shifts and occasional mid shifts. These are typically eight hours shifts. When applying, candidate location and availability will be taken into consideration as well as the consideration of program needs. The location of these programs are mostly located in Clackamas County, Oregon City area, with the exception of one in Wilsonville and one in NW Portland. As Cascadia is Trauma informed, the incumbent must support a Trauma Informed Care Environment. With the high prevalence of trauma among individuals receiving behavioral health services, it is required that the incumbent understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. Essential Duties: This position description is not intended to be an all-inclusive list of responsibilities or an all inclusive job description. Provide support with activities of daily living such as self-care, grooming and accompany residents to appointments if necessary. Lead skills training groups: I.e. cooking groups, writing groups, social skill groups, etc. (the fun part is bringing in your creativity) Assigned to one or two residents as a Primary Treatment Coordinator and you will have the opportunity to work with Case Managers in supporting residents with their support plans. Overnight positions may have the opportunity research and learn more clinical knowledge or terms and will provide more support in house hold chores since this can be a quieter time. Ultimately, it will depend on the program needs. Help with household chores Medication management and medication documentation Electronic record documentation that will includes progress notes as well as any notes or reports pertaining to critical incidences Assist in crisis situations as appropriate, following emergency protocols and procedures. Have the ability and be prepared to assist with avoidance preventing escalation. Demonstrate professional boundaries, abide by HIPPA, and complete required annual trainings. Perform other duties as assigned Qualifications Level I requires no experience Level II requires a QMHA: a Bachelor's degree in a behavioral science field from an accredited college or university or three years of full time professional experience working in the mental health related field. (QMHA qualified required) Note: All QMHA positions in the state of Oregon will need to be filled by candidates who are officially QMHA certified or registered with MHACBO. For details, go directly to MHACBO Having experience working in a residential facility providing direct support is helpful Experience working with those living with a severe and persistent mental illness is desired Ability to operate computer hardware and software and ability to learn and use electronic health record systems Working Conditions Staff must have the ability to assist residents in participating in a variety of active leisure and recreational events, as well as skill building outings and accessing community resources. These are required to avoid behavioral incidents and volatile situations. Must have the ability to assist the team in providing safety to all clients and staff in potentially volatile situations.
