SUMMARY
The primary goal of the facility-based Advanced Practice Provider (APP) is to deliver direct patient care services at the Provider level in a Post-Acute Care environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Adopts our company's ethos to provide care that fosters the preservation or improvement of each patient's clinical condition and quality of life.
- Conducts Annual Wellness Visits for all long-term Nursing Facility residents and may perform Annual History and Physicals upon request by Facilities.
- Performs clinical interventions and episodic direct care visits and conducts appropriate re-evaluations.
- Provides continuous assessment and treatment for chronic co-morbid conditions, as deemed medically necessary.
- Assesses and treats residents for changes in their health status.
- Reviews medications, orders, and diagnostic tests and responds to clinical anomalies for all patients under their care, as permitted and/or requested by collaborating physicians.
- Collaborates with physicians as needed for comprehensive medical management.
- Participates in the management of facility-wide illnesses when appropriate collaborations are established.
- Coordinates care with various healthcare disciplines, including Therapists, Specialists, and Social Workers.
- Maintains communication with families and attends family conferences whenever feasible.
- Performs other duties as assigned.

QUALIFICATIONS
- Holds a valid nurse practitioner or physician assistant license in the state of operations and maintains current board certification in their respective field.
- Possesses a minimum of a Bachelor's and Master's Degree in Nursing from accredited Colleges or Universities.
- Nursing experience is mandatory, and some prior experience as a Nurse Practitioner/Provider is preferred, though not obligatory; however, demonstrated critical thinking skills are a prerequisite.
- Demonstrates knowledge of Nursing and the application of critical thinking skills for Nurse Practitioners in caring for elderly, chronically ill, and convalescent residents.
- Exhibits strong leadership, interpersonal, and problem-solving skills, which are highly desirable.
- Displays excellent written and oral communication skills.
- Demonstrates punctuality, organizational abilities, and the capability to work effectively within a team.
