Position Description Overview
The Nurse Practitioner or Physician Assistant reports to the Senior Manager Clinical Care and VBC with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. Integral to the primary care team, you will perform annual health assessments, complete post discharge visits, and deliver cost-effective, quality care to assigned patients. Working collaboratively across a multi-disciplinary team, you will contribute to the development, implementation, and on-going refinement of home health processes.

Job Responsibilities
The Nurse Practitioner or Physician Assistant will have the following responsibilities:
  • As the Primary Care provider the Advanced Practice Provider will be Performing comprehensive in-home assessments for identified patients of the practice and manage chronic diseases
  • Reviewing current symptoms and identifying diagnosis to be used in care management and active medical management of treatment
  • Completing physical examinations, medication reviews, vital checks, lab draws, wound care and depression screenings
  • Using their assessment findings to identify and address potential gaps in care
  • Accuracy on risk capture, high quality metrics and closure rates
  • High patient satisfaction and improved health status including reduced hospitalization and ER visits
  • Thinking critically to address patient needs, providing the appropriate resources and follow-up care to improve stability
  • Educating patients on topics such as disease process, medication, and compliance
  • Complying with all HIPAA regulations and maintaining security of protected health information
  • Ability to travel 5 days a week to provide care to patients in their home based on the needs of business/market
General Duties
The Nurse Practitioner or Physician Assistant will have the following duties:
  • Leadership : The Nurse Practitioner or Physician Assistant will lead in defining and executing strategies and solutions to create business value, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes.
  • Strategy : The Nurse Practitioner or Physician Assistant will establish the business strategy and roadmap: (1) improve outcomes for Family Members; (2) enhance the efficacy of other CINQCARE. business divisions; and (3) develop and deliver external market opportunities for CINQCARE products and services. In establishing the business strategy, the Nurse Practitioner or Physician Assistant will define and innovate sustainable revenue models to drive profitability of the Company.
  • Collaboration : The Nurse Practitioner or Physician Assistant will ensure that capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQCARE.
  • Knowledge : The Nurse Practitioner or Physician Assistant will provide subject matter expertise in the solutions, including determining and recommended approaches, solution deployment and performance evaluation.
  • Culture : The Nurse Practitioner or Physician Assistant is accountable for creating a productive, collaborative, safe and inclusive work environment for the team and as part of the larger Company.
Qualifications
The Nurse Practitioner or Physician Assistant should have the following qualifications:
  • FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA
  • Experience: 2 + years of clinical experience as an NP/PA in the primary care or home care setting
  • Current state license
  • Nurse Practitioner certification from either the ANCC / AANP
  • Working knowledge of chronic care management and various medication options
  • Current DEA licensure / Prescriptive Authority post-hire
  • Experience in Family Medicine, Internal Medicine, Gerontology, Palliative Care
  • Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)

  • Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCARE's team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.
