Job Description: As an APP Hospitalist at Jersey Community Hospital, you will play a vital role in providing comprehensive medical care to our patients. Working under the supervision of our physician hospitalists, you will have the opportunity to manage patient care in a collaborative and supportive environment.
Key Details:
- Patient Load: Average of 9 patients per day
- Physician Tele-Oversight: Collaborative oversight from physician hospitalists
- EMR: CPSI
- Hospital Capacity: 31 beds, including an open ICU (hospitalists round on ICU patients)
- Shifts: 24-hour shifts (rounding plus call)
Responsibilities:
- Conduct comprehensive patient assessments, including medical history review, physical examinations, and diagnostic evaluations.
- Develop and implement evidence-based treatment plans in collaboration with physician hospitalists.
- Provide ongoing patient monitoring, management of acute and chronic conditions, and coordination of care transitions.
- Perform procedures within scope of practice, including but not limited to, venipuncture, arterial line insertion, and lumbar puncture.
- Communicate effectively with multidisciplinary teams, patients, and families to ensure coordinated and patient-centered care.
Requirements:
- FNP/ PA Certification Required
- Active, unrestricted license to practice in Illinois
- DEA/ CSL- Required
- ACLS (AHA), PALS (AHA- Required
- Strong clinical skills and ability to work independently
- Excellent communication and interpersonal skills
