This position will be responsible forlinking mobile resources together, integrate and coordinate them torespond appropriately to patient needs. Telephonic patient triagefollowing established evidence-based protocols to assist innavigating care across the health care continuum. Responsible fortelemonitoring and patient education activities. Activelycoordinates team care in a virtual setting. Additional dutiesinclude leadership roles and mentoring asassigned.

ESSENTIALFUNCTIONS/DUTIES:
Demonstrates theability to function as a Registered Nurse.
Telephonically triages patients and navigates care based uponevidence-based protocols.
Patient education across thehealth care continuum.
Telemonitoring activitiesengaging healthy patient behaviors.
Must demonstrateeffective communication skills and be able to work well with othersincluding team members, patients, caregivers, referral sources,physicians and their staff, facility staff members and thecommunity.
Demonstrates ability to effectively learn anduse computer and required software programs
Participatein activities that promote the AMR Integrated Healthcare Programand the AMR organization at large.
Must be able to actas a facilitator and educator for the Integrated Healthcaretraining course provided by AMR.
Must demonstrate theability to manage time, resources and paperworkrequirements
Provide excellent customerservice
Perform other duties asassigned

QUALIFICATIONS:
5 years or more of RN experience in Emergency or Acute Caresetting
Experience with clinical data gathering andEthical Principles.

EDUCATION:
Graduate of an accredited school ofvocational nursing
Master's or Bachelor's degree inHealthcare Administration, Public Health or any other degree with afocus on community or diseaseeducation;

SKILLS:
Strong and effective verbal, written, and interpersonalcommunication skills.
Nursing skills as defined asgenerally accepted standards of practice. Good interpersonalskills.
Ability to adhere to established standards foreducational quality.
Familiar with basic computerapplications and functions.
Proficient Bilingualtranslations skills
Demonstrated teaching andeducational facilitation skills. Credentials/Licenses:
Maintain current NVL state Nurse license with ability to obtainmulti state licensure
Current CPRcard.
