This Nurse Navigator is responsible for assisting with coordination of the care provided to the patient, and communication of plan of care is given in an appropriate and timely fashion to the patient's family. Assists the care team with patient and family communication; and ensures optimal care and providing the patient, family and caregivers with the support and knowledge they need. The Family Connections Navigator fulfills the primary responsibilities for coordinating a multi-disciplinary approach in the care team-patient-family communication loop up until the patient is discharged from the emergency department. This position also exhibits skills as an empathetic patient advocate and skilled communicator and serves as a representative for the care and treatment plans. By working with all applicable departments, this position will ensure that all patient communication, education and coordination be consistent and aligned with and/or an extension of the providers' recommendations. ASN required; BSN preferred. Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license. Basic Life Support certification required. 3-5 years of nursing and/or clinical care coordination preferred. Emergency Department experience strongly preferred; triage or charge nurse experience preferred. Technological capabilities to coordinate virtual communication with patients and families (i.e. e-mail, virtual visits) required.
