Nurse Manager, RN

POSITION SUMMARY

The Nurse Manager is accountable for the management and leadership of a specific nursing unit. This position evaluates available resources and delegates appropriately to assure quality outcomes of care. They also directs and implements the nursing process. This includes systematic data collection, assessment, development of care plan, implementation of care plan and revision of plan. The Nurse Manager manages and supervises the care of residents through a well-defined care delivery system

ESSENTIAL RESPONSIBILITIES AND DUTIES RESPONSIBILITY: To Facilitate the Comprehensive Care Planning Process
  • Systematically collects data utilizing the MDS about residents through interview of resident and family, observation, discussion with physician and review of medical records.
  • Assesses residents and applies knowledge to facilitate the best therapeutic plan of care. May use nursing diagnosis as relevant.
  • Assists and is knowledgeable in the development of care plan using the CAAS in collaboration with the interdisciplinary team.
  • Implements care plan by assigning responsibility, developing communication systems and following up on outcomes.
  • Attends care conferences meeting the needs of the resident and family.
  • Monitor any change of condition and revises the plan of care to reflect the current resident status.
  • Assists in the monitoring for changes of condition and revises the plan of care to reflect the current resident status.
  • Assist and complete assessments. Supervises the completion of the functional care plans by other nursing personnel.
RESPONSIBILITY: Has Knowledge of and Consistently Applies Established Nursing Policies and Procedures According to State and Federal Regulations and Nurse Practice Act.
  • Is knowledgeable of and abides by the residents' Bill of Rights.
  • Uses standards of care and specific policies to perform nursing care procedures for residents.
  • Allows and encourages autonomous decision making to resident whenever possible.
  • Abides by and promotes the Continuous Quality Improvement program of the Rehabilitation Center
  • Is involved in making necessary recommendations and changes to nursing administration that directly And indirectly affect care given to residents
  • Attends in-service and reviews literature to maintain skills in gerontological nursing.
  • Draws lab or delegates this as appropriate.
RESPONSIBILITY: Supervises and Manages RN's, LPN's and CNA's. Manages all Staff When in Charge of Building
  • Articulates expectations and delegates appropriate job duties to subordinates, using clear and concise communication.
  • Follows up on directives given to subordinates and assures completion of duties.
  • Complete performance appraisals as delegated by the DON.
  • Assesses knowledge and skill of supportive nursing personnel to assure safe practice of nursing staff.
  • Confronts, corrects and explains unprofessional behavior to these staff.
  • Prioritizes and updates work assignments consistent with the number and levels of staff available that shift and acuity of residents care necessary.
  • Take necessary steps to replace appropriate nursing staff to maintain proper staffing level. This may include working as the staff nurse.
  • Knows and can implement fire, tornado and disaster plans.
  • Works designated weekends and takes call as directed by the DON.
RESPONSIBILITY: Has Knowledge and Consistently Follows Standards of Documentation and Written Communication
  • Completes charting in accordance with health information standards and facility policy, particularly for residents on Medicare Part A and those with behaviors.
  • Resident assessment data is systematically collected, is communicated through report, and is clearly documented in the medical record.
  • Gathers and documents data on admissions and discharges.
  • Is knowledgeable in the procedure to facilitate physician orders and completes procedures to assure accurate follow-up.
  • Is knowledgeable in and initiates reports and documents nursing orders.
  • Provides report to CNA's and other professional staff, verbally or written about daily resident care needs and/or significant changes in condition.
  • Follows up on documented recommendations of the dietitian.
RESPONSIBILITY: Complies with Pharmaceutical Policies
  • Administers medications accurately according to policy, including handwashing and 5 rights.
  • Complies with standard of remaining 95% free of medication errors.
  • Signs out medications and treatments including PRN's according to policy.
  • Reviews medication and treatment sheets monthly.
  • Uses proper procedure for label changes.
  • Follows proper emergency kit procedures.
  • Follows control substance policy.
  • Destroys controlled medications with pharmacist and non-legend drugs with the DON.

FACILITY COMPLIANCE

  • Promotes a highly positive workplace centered on strong customer service, inclusivity and ensuring staff feel welcome and supported
  • Provides Rehabilitation Center with current license and keeps updated.
  • Attends continuing education/in-services pertinent to your department.
  • Attends all staff education of the Rehabilitation Center relating to safety, infection control and disease process.
  • Attends educational programs on Continuous Quality Improvement, customer satisfaction and other such programs. Participates in inspections and audits as designated.
  • Maintains current knowledge of Minnesota Department of Health regulations.
  • Identifies areas of problem/needs through surveys and communicates these at QA&A Committee meetings.
  • Ability and willingness to work some evenings and weekends as directed by supervisor.
QUALIFICATIONS Education and Experience
  • Must have current registration, as a Registered Nurse, in the State of Minnesota.
  • Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
  • Must possess the ability to communicate verbally and in writing when directing the care of residents.
  • Must be able to perform essential functions of the position with or without reasonable accommodation.
  • Must qualify for employment, after criminal background study, per guidelines of Minnesota Department of Human Services.
Preferred
  • Experience in healthcare, skilled nursing facility or assisted living community
  • Bachelor of Science Nursing Degree
