Full job description

The LPN/RN is responsible for providing direct patient care under the supervision of the Director of Nursing or physician. The LPN/RN will be responsible for administering medications, taking vital signs, performing wound care, and assisting patients with daily living activities. The LPN/RN will also be responsible for documenting patient care in the electronic health record system and liaising with other healthcare professionals as needed.

Responsibilities

  • Administer medications as prescribed by a physician or Director of Nursing
  • Monitor and record patient vital signs, such as blood pressure and heart rate.
  • Communicate with patients and their families about their care and treatment plan.
  • Document patient visits and treatment plans in the electronic medical records system
  • Collaborate with other healthcare professionals, such as registered nurses, physicians, and therapists.
  • Complete patient assessments, coordination of care
  • Identify and report changes in the resident's condition.
  • Follow infection control and safety protocols to ensure patient and staff safety.
  • Maintain confidentiality and ethical standards in all patient interactions.
  • Other duties as assigned by management.

Requirements

  • Valid Illinois Nursing License LPN or RN.
  • Experience in long-term care or healthcare setting preferred.
  • Excellent communication and interpersonal skills.
  • Strong attention to detail and ability to follow instructions accurately.
  • Ability to arrive on time for all scheduled shifts.
  • Must have the ability to read, write, and follow oral and written directions at a level necessary to accomplish the job.
  • Detail-oriented with strong organization and prioritization skills.
