Nurse Lebanon

Be part of a dedicated and compassionate team of caregivers. West Central is seeking a full-time Nurse to work in our outpatient facilities in Lebanon and Claremont, NH. Responsibilities: Evaluates client's medication status, compliance and side effects at each visit. Coordinates higher level of care for clients when additional care is needed. Problem-solves with clients to reduce issues that interfere with medication compliance. Coordinates treatment with primary care provider, as well as psychiatrist. Triages client phone calls, answers questions, and refers problems to agency psychiatrist. Completes all clinical documents. Stays current with evidence-based practices through supervision, trainings and independent readings, etc. Education: BSN, RN or LPN licensed in the State of New Hampshire Experience: Minimum of 3 years of psychiatric nursing, preferably in an outpatient setting Required Skills: Excellent organizational and communication skills Excellent customer service skills Good computer skills and knowledge of Microsoft Office Knowledge of and experience with electronic medical records
