National Health Corps: Community Health Fellowship of NEPA National Health Corps (NHC) Community Health Fellowship is an AmeriCorps National Service program that recruits, trains, and places individuals in resource-limited organizations in their communities called "host sites." These NHC members help build healthy communities by providing essential health education and access services to individuals in communities with histories of racial and economic inequities, while while receiving learning opportunities with health, public health and equity experts , and workforce development in three key public health roles community health worker, digital health navigator and/or medical interpreter. Further help on this page can be found by clicking here. Member Duties : 1. Working with the geriatric team on resources for patients and caregivers to plan for resources and support services. Planning education groups. 2.Establish help with housing - not only for homeless elders but transitions to housing as their care needs change. Working with the doctor and the nurses and the caregiver as a team to be sure the patient is safe in their home environment. Home evaluations to see if the patient requires home medical visits 3. Assisted Living and Home health - Gather different brochures for patients and caregivers 4.Assisting patients to get into programs such as Adult Day services / Senior Centers- 5. Work with Area Agency on Aging 6. Support services for the caregivers - connect caregivers to resources for respite as appropriate 7. Complete phone surveys on Social Determinants of Health.
