Mental Health Worker - Clara Maass Medical CenterReq : Category:Behavioral and Mental Health Status:Full-Time Shift:Day Facility:RWJBarnabas Health Corporate Services Department:QIP Location: SBC Corporation, 1 Clara Maass Dr, Belleville, NJ 07032 QIP BEHAVIORAL HEALTH SPECIALIST GENERAL DESCRIPTION: The Mental Health Specialist will assist in navigating patients in need of mental health, substance abuse, primary care services and ensure the patient s social determinants of health issues are addressed. The Mental Health Specialist acts as a contributing member to the interdisciplinary team at various access point sites including the Emergency Department, PESS/Crisis units, inpatient units and community providers. The Mental Health Specialist will initiate post discharge follow-up for mental health services and support adherence to the aftercare appointment. LOCATION & HOURS: Clara Maass Medical Center in BellevilleWould be cross-trained and expected to provide coverage at other RWJBH hospital facilities on an as-needed basis. Shifts: Monday-Friday RESPONSIBILITIES AND DUTIES: + Meet the patient in the Emergency Department, PESS/Crisis units, Inpatient Units and in the community to engage the patient and navigate to the proper follow-up location + Initiate post discharge follow-up for mental health services and support adherence to the appointment and communicate the discharge plan with the interdisciplinary team + Assist patients eliminating barriers to kept appointments such as, arranging for transportation and/or transporting the patient via company vehicle + Provide motivational interviewing interventions and provide timely feedback regarding the patient s care, treatment recommendations and progress via documentation in the client s record and verbal feedback. + Gather patient information to complete required documentation + Ensure that assessments and other documentation required to support NJ-QIP program requirements are completed and follow up to ensure all data is collected in a timely manner + Ensure effective communication with the entire team + Foster relationships with key stakeholders + Partner with multidisciplinary team during the development of relapse prevention plans + Prepare status reports and other administrative duties as needed for QIP-NJ initiative. + Conduct outreach to patient and community partners as needed + Performs all other duties as assigned. QUALIFICATIONS: + High School Diploma or GED equivalent; BA in Social Work preferred + Prior experience as medical office assistant, medical assistant, or medical access/call center a plus + Must have reliable transportation + Must have skills with the ability to email, navigate Microsoft Work, and other software systems. + BLS and successful completion of required training modules within 30 days of hire + Strong interpersonal, communication, organizational, time management skills are key + Bilingual a plus DIRECTION/SUPERVISON RECEIVED: Director of Operations, QIP-NJ Behavioral Health Program RWJBarnabas Health is an Equal Opportunity Employer
