We are seeking a community-based Mental Health Professional to join our team in our Bethlehem, PA location. Position Details acts as an advocate for a caseload of clients by supporting their integrated health care needs, both physical and behavioral. Utilizes your own vehicle (company mileage reimbursement) while meeting with your caseload daily in their homes, community, and at appointments. Coordinates care throughout medical and psychiatric hospitalizations, including aftercare and follow-up requirements. Develops service plans with time limited goals to support clients to increase independence. Conducts assessments with clients to determine outcomes and assist clients with coordinating appointments. Offers assistance in medical and behavioral outpatient services. Emphasis on linking clients to community-based resources. Educates clients and their natural supports about mental health and medical diagnoses. Assists with client's insurance coverage, lab work, and medication compliance. Focuses on holistic approach to wellness while acting as a liaison between client's medical and behavioral health treatment providers. Earn $56, 000 / year. $2000 Sign-On Bonus available. Experience in mental health field and co-occurring/substance use is preferred. Valid driver's license required. Open to a community-based role
