Job Description:
The Integrated Care Therapist will be responsible for providing mental health assessment, crisis intervention, safety planning, brief therapy (up to three months), care coordination, and other services as part of the Mental Health staff integrated within the primary care teams. All duties will align with the Yellowhawk Mission and Vision statements.
