Also, provides outreach to the community and participates in community education. Essential Duties and Responsibilities This job description reflects management's assignment of essential functions; it does not prescribe or restrict the tasks that may be assigned. Perform Medicare Part D drug assessments by entering consumer's drug information into the web-based Medicare Part D plan finder and providing consumer with options presented by the plan finder. Process SeniorCare or Medicaid applications including assisting the consumer with gathering consumer-specific information and/or completing the SeniorCare or Medicaid application. Perform data entry into the SHIP data base. Problem solve issues relating to Medicare Part D and SeniorCare, including looking at consumer-specific situations to determine why consumer is not receiving the full benefit from their drug plan or providing clarification to consumer on benefit of chosen drug plan. Perform outreach to clinics, pharmacies, etc. including providing pharmacies with information to share with their customers and to educate pharmacists on Medicare/Medicare Part D and how their customers can benefit (i.e. brochures, Medicare Workshop announcements, Medicare Updates, etc.). Participate in Medicare workshops by assisting with consumer education and assistance with set-up/planning as needed. Perform home visits to home bound consumers regarding SeniorCare or Medicare Part D issues, including completing a Medicare Part D drug assessment, SeniorCare application, problem solving on Medicare Part D or SeniorCare, etc. Demonstrate a commitment to county safety and risk management efforts. Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience Associate's degree, preferably in a health and human services related field and two to three years of relevant prior experience. Proven experience in ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals as well as ability to compute rate, ratio and percent and to draw and interpret bar graphs. Knowledge of Trauma Informed Care principles is essential. Interactions and Communications Requests are answered and contacts initiated to exchange basic and/or general information. Certificates, Licenses, Registrations Valid Wisconsin driver's license. Proof of minimum liability auto insurance coverage.
