Overview: We are seeking a dedicated Medical Social Worker to join our healthcare team. This position primarily involves fieldwork, including visits to patients' homes. In this role, you will provide vital support to patients and their families by addressing their psychosocial needs and advocating for resources to promote their overall well-being. The ideal candidate will possess strong communication skills, empathy, and a deep understanding of social determinants of health. This position offers the opportunity to make a meaningful difference in the lives of individuals facing medical challenges. Responsibilities: Conduct comprehensive psychosocial assessments of patients to identify their needs, strengths, and challenges. Collaborate with interdisciplinary teams to develop and implement individualized care plans that address patients' psychosocial concerns and support their medical treatment. Provide counseling, emotional support, and crisis intervention to patients and their families facing medical, emotional, or social difficulties. Assist patients and families in accessing community resources, such as financial assistance, housing, transportation, and support groups, to address their social and practical needs. Advocate for patients' rights and facilitate communication between patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes. Maintain accurate and confidential documentation of patient assessments, interventions, and progress in accordance with professional standards and regulatory requirements. Stay informed about social and healthcare policies, regulations, and resources to effectively support patients and advocate for necessary changes or improvements in service delivery. Qualifications: Master's degree in Social Work from an accredited program. Current state licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent. Strong knowledge of social work principles, theories, and interventions in a healthcare setting. Excellent communication, interpersonal, and problem-solving skills. Ability to work independently and collaboratively with a team. Compassion and sensitivity to the needs of patients and families. Proficiency in using electronic medical records (EMR) and other relevant software applications. Bilingual / Spanish fluency is strongly preferred.
