The Social Worker will utilize their advanced educational background, their clinical expertise, judgment and discretion, to help identify the patient and family's unique cultural/spiritual beliefs, coping skills, pre-bereavement and anticipatory grief needs to offer a wide range of services to terminally-ill patients and families. Develops and implements individualized plans of care including creating measurable patient-centered goals in collaboration with the patient and family. Reviews and updates the goals and interventions of the plans of care as well as the visit frequency based on ongoing assessments in order to meet the patient's changing needs. Direct patient care will be the priority and productivity will be monitored by the manager. Participates as a member of the hospice team in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Team (IDT) meetings, to assist with the coordination of services. Coordinates and documents collaboration between Ohio's Hospice, contracted providers and community agencies to facilitate linkage of services. Gathers and documents patient's demographic and contact information of family members for Bereavement purposes. Ensures quality communication with the patient's Interdisciplinary Team by thoroughly documenting and updating assessments, goals and interventions in the electronic health record at the point of service and then synchronizing this documentation for each patient according to Ohio's Hospice standards. Assists in other activities and departments when requested to ensure complete and thorough patient care for all Ohio's Hospice patients. Participates in Continuous Quality Improvement activities to support the quality of Ohio's Hospice services. Precepts new staff to support professional relationships with newly hired team members as required. Qualifications: Licensed Social Worker with the state of Ohio LISW preferred or LSW. Master's Degree in Social Work or related degree as required for LSW licensure. One year social worker experience in a health care setting preferred. Computer skills sufficient to properly document services and care. Able to drive during daytime, nighttime or inclement weather. Valid Driver's License with Safe Driving Record. State Minimum Automobile Insurance coverage. Notary designation or intention to complete designation within six months of hire. As a member of our team, you will have a chance to impact many lives.
