• Develop and implement customized care plans for each client based on their specific needs and preferences
• Conduct in-home assessments to evaluate clients' health conditions and recommend appropriate services
• Coordinate with healthcare professionals, family members, and other caregivers to ensure a comprehensive care approach
• Maintain accurate and up-to-date documentation of client interactions, care plans, and progress
• Provide compassionate and respectful care to enhance the quality of life for clients and promote their independence Qualifications:
• 1+ years of experience in healthcare Social work
• Strong organizational and time management skills
• Proficiency in electronic medical records systems
• Excellent verbal and written communication abilities
• US Work Authorization
• Must be Licensed
