Description:

Statement of Purpose: While living healthy and productive with an HIV diagnosis, being able to assit others navigate the challenges of living with HIV. Create rapport, share experiences and access patients' needs. Provide education and use a strength base approach to improve patients' adherence to medical protocol, coping skills, and quality of life. Liaise with the Medical Teams, Case Management, Patient Care Coordinators, etc. Must maintain compliance with HIPAA.

Primary Tasks:

  1. Must be comfortable disclosing HIV status to clients and relevant staff.
  2. Implementation of the core values of the Medical Peer Navigator program.
  3. Assess and address clients' knowledge, attitudes and beliefs regarding HIV.
  4. Assess patients' adherence, willingness, and abilities regarding antiretroviral therapy.
  5. Assess patients' needs and create referrals for services including dental, mental health, and case management.
  6. Assist patients with strategies to overcoming barriers to care using a strength base approach.
  7. Educate patients about support groups, educational opportunities, etc.
  8. Address a prevention care plan to improve overall health.
  9. Review case notes, labs, and medical encounters to coordinate care.
  10. Provide HIV and sexual health education regarding HIV and sexually transmitted diseases.
  11. Documentation of all patient interactions and activities in EMR system within 48-hours.
  12. Communicate with medical team, case management, etc. through CAN approved technology.
  13. Plan, attend, and facilitate Peer related groups and events.
  14. Ability to work with a multidisciplinary team.
  15. Educate patients about the 340B Program and their pharmacy choices.
  16. Participate in QAQI, Interdisciplinary care, and other relevant meetings.
  17. Maintain knowledge of medical terminology and practices pertinent to HIV care.
  18. Follow-up with no-shows and contact patients with appointment reminders when needed.
  19. Submit reports, payroll, and other documents as instructed by the manager.
  20. Promotes and practices CAN Community Health, Inc.'s mission and values, and follows its policies and procedures.

Secondary Tasks:

  1. Maintains confidentiality is maintained regarding patient and/or family in accordance with HIPAA and with professional and departmental standards.
  2. Participates in continuous quality improvement of services rendered to CAN Community Health patients.
  3. Develops and fosters networking relationships with community and support resources.
  4. May accompany patients to appointments for specific reasons with approval from management.
  5. Participation in the HIV Speakers Bureau is encouraged.
  6. Stay up-to-date with the new data/science in the field of HIV.

Requirements:

Knowledge, Skills and Abilities Required:

  1. Adept at navigating cultural differences.
  2. Adept with working with persons of color, gender and the lesbian/gay/bisexual/transgender/queer community (LGBTQ+).
  3. Understanding on how HIV is transmitted and prevented as well as familiarity with antiretroviral therapy.
  4. Ability to work independently with accountability.
  5. Exercise sound judgment, discretion and professionalism.
  6. Sound communication and interpersonal skills with patients, visitors, fellow staff members and the supervisory team.
  7. Bi-lingual(English/Spanish) is preferred.
  8. Familiar with basic computer skills and use of social media.
  9. Working knowledge of Microsoft Office products.
  10. Demonstrated oral and written communication, organizational, multi-tasking and problem-solving skills, ability to work under pressure to meet deadlines.
  11. Able to work with minimal direct supervision.
  12. Ethical conduct.

Physical Demands:

  1. Neat, professional appearance.
  2. Requires full body motion including handling and lifting patients.
  3. Medium- Exerts up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. force constantly, to move objects, including the human body. Involves sitting, walking, and standing for brief periods of time.
  4. Requires expressing or exchanging ideas by means of spoken word, visual and auditory acuity.
  5. Valid driver's license and ability to operate a motor vehicle.
  6. Travel between facilities and outside agencies.

Education/Professional:

  1. Experience as an outreach worker or case manager is preferred.
  2. Relevant HIV experience is required.

  1. Computer equipment
  2. Multi-line telephones
  3. Fax & other business machines and/or technology.
