This opportunity is for a full-time (3.5 days per week) post-acute care medical director to join our well established Connecticut post-acute team. In this role you will work in a skilled nursing environment providing primary care services. This exciting opportunity offers professional autonomy, a flexible schedule and uncapped earnings. Position Overview: Provide primary care services in post-acute care facilities including skilled nursing and assisted living to a patient population of long-term care, short-term rehab, memory care, and clinically complex patients Drive high quality, safe and cost effective care of patients in our local post-acute care communities H&P, admission, progress, transfer, discharge, and acute/follow up Work closely with hospice, palliative care, and home care agencies to ensure a safe transition home Document via our EMR (Gehrimed) in a timely fashion Provide oversight, guidance, and mentorship to our clinicians within your facilities Conduct clinical case reviews as needed and share tools, information, and guidelines as they relate to quality of care Collaborate with leadership to review care coordination initiatives and develop collaborative intervention plans Background in primary care, internal medicine, family medicine, emergency medicine, urgent care, physiatry, PM&R or geriatrics preferred Monday through Friday schedule with flexible daily hours No overnight call; 7pm to 7am call coverage is facilitated by our APC call team Compensation includes a base salary, productivity increase potential and directorship stipend Why TeamHealth?
