• An employee recognition program Responsibilities Plan, organize, develop and coordinate MDS Assessment Process. Assure compliance with all state and federal MDS transmission requirements. Collaborate with clinical and administrative staff regarding issues relative to the Patient Assessment Instrument (PAI) process. Educate unit staff regarding OBRA regulations, and serve as a resource regarding the policies and procedures of the skilled nursing facility. Required Qualifications Core Competencies: Protect patient confidentiality by promoting professional staff communication according to HIPPA standards. Adhere to established Center and departmental policies, procedures and objectives. Exemplify professionalism and exhibit values, which contribute to the achievement of the mission of Hebrew SeniorLife. Perform other related duties as required or as responsibilities dictate. Position Responsibilities: Review hospital or transfer records in accordance with procedure to determine rehabilitation potential of patient's condition. Screen all admissions based on age, diagnosis and functional status of admission to RSU. Perform an accurate MDS assessment of each RSU patient within required time frames, which optimizes appropriate reimbursement. Devise and coordinate a compliance program for all submission requirements based on time frames. Maintain appropriate records: Initial Feedback Reports, Validation Reports, and Quality reports. Responsible to transmit to CMS all mandated RAI forms, including MDS, Quarterly Review Forms, Discharge and re-entry Tracking Forms and Face Sheet information. Transmit MDS batches to the CMS after checking for accuracy and addressing any and all error alerts. Analyze feedback and validation reports and provide follow-up to administration and staff. Provide edit checks and make necessary changes. Conduct weekly chart audits to insure that staff observations and records accurately reflect the level of care provided. Provide on-going education to RSU staff regarding FAI compliance issues, maximization of appropriate reimbursement and Medicare program requirements. Act as a resource for MDS / Medicare questions. Report issues of non-compliance to Senior Director Post-Acute Population Health and Transitions of Care Actively participate in "TEAM" conferences. Assist in maintaining compliance with all DPH and federal regulation. Perform all duties in an independent manner with little or no need for direct supervision. Act as a resource to all levels of staff in areas pertaining to SNF unit education. Assist in assessing learning needs of unit. Plan, implement, direct and evaluate orientation of new primary team personnel. Provide formal and informal ongoing education to the primary care team. Develop, teach, and evaluate educational programs collaboratively with Nursing Professional Development Act as liaison with Medical staff, other departments, patients and family members to facilitate admission to the unit. Preferred Qualifications Qualifications Graduate of an accredited school of nursing. BSN preferred. Current Massachusetts license as an RN. Computer literacy required. Demonstrated organizational skills. Extensive knowledge of RAI process. Extensive knowledge of pertinent state and federal regulations.
