Spring Creek Nursing and Rehabilitation is a beautiful campus located in Green Springs, OH. Spring Creek strives to have an unparalleled reputation for programs, services, and exceptional care. We provide specialized care services for patients requiring extended medical care and rehabilitation. Those with catastrophic or acute illnesses and injuries can work towards recovery. We also offer both short-term rehabilitation and long-term skilled nursing care, along with a full respiratory unit that provides ventilator care and all respiratory needs.
Due to our growing censues, Spring Creek i s adding an additional team member to our nursing deparment. Spring Creek is currently seeking an experienced professional to fill a part-time Minimum Data Set (MDS) nurse position (possiblity of moving to full-time). Interested candidates must be have a valid Ohio Nursing License (RN preferred) and at least one year of MDS experience.
Job Responsibilities are but not limited to the following:
-Assure compliance with current rules and regulations as mandated by the Ohio Department of Health and PPS schedules mandated by Centers of Medicare and Medicaid Services
-Conduct and coordinate the development and accurate completion of the resident assessment (MDS, and Care Plans) in accordance with current rules, regulations and guidelines
-Communicate appropriate information to the Interdisciplinary Team/Management Team, and participate in QAPI process as appropriate.
-Assure completion dates are set and entered on the Resident Care Conference Schedule, resident interviews are scheduled/completed, and resident/responsible party notices for Resident Care Conferences are sent in a timely manner.
- Ensure accurate and timely completion of assessment and forms related to the MDS including, but not limited to, Braden, Skin Grids, Admission, Falls, Restraints, Side rails, AIMS, Bowel & Bladder, etc.
Qualified candidates must possess a current Ohio Nurse license (RN preferred), plus have 1 years of experience in MDS process.
