MAT Co-Care Nurse Care Manager - Posting

Hourly Rate: $43 - $48
Annual Salary: $90,000 - $100,000

Position Summary:

Full-time Medication Assisted Treatment (MAT) Co-Care Nurse Care Manager position available for our Vancouver Medical Clinic, with travel to the other Sea Mar medical clinics in Clark County. Structured as a program focused on individualized patient care targeting reduced harm to the patient and community, the NCM supports the MAT Program and its goal of assisting patients engaged in a whole person recovery from substance use disorder(s). The NCM's tasks and responsibilities will be associated with the National Institute on Drug Abuse (NIDA) CTN-0139 Collaborative Care (Co-Care) clinical trial study occuring in the Vancouver region.

This study duration is 2.5 years beginning in June of 2024. Sea Mar Seeks to retain the position beyond the length of the study, however, this will depend on the regional need and patient demand.

Responsibilities include:
  • Demonstrate knowledge of addiction as a chronic brain disease as well as evidence-based interventions such as Medication Assisted Treatment for people living with substance use disorder.
  • Active engagement with people who use illicit substances using a team-oriented collaborative, person-centered approach grounded in harm reduction, trauma informed care and healing based practices.
  • Provide ongoing addiction treatment and follow up to patients. This includes, but is not limited to, facilitation of intake assessments, program orientation, induction, stabilization, recovery maintenance, and relapse prevention.
  • Demonstrate knowledge and skills necessary to provide appropriate care to the patients being served. The Nurse Care Manager will be required to function as a consultation nurse, perform nurse visits, and work directly with MAT providers according to the specific needs of the patient and the MAT program.
  • Team oriented: Coordinates care with other MAT Program staff and MAT Providers via the MAT Clinic huddle and otherwise as needed. Works collaboratively with Sea Mar administration and agency staff at large to secure the resources need to best equip the patient in attaining success.
  • Ensure that Sea Mar policies are followed, noting all deviations and instances where policy contradicts or impedes patient care and reporting all observations and findings in writing to the Program Director immediately.
  • Coordinating between the MAT provider and pharmacy regarding initial prescriptions and prescription refills for MAT Program related medications. This may include providing assistance with prescription processing and refills, prior authorizations, insurance issues, concerns of diversion, patient safety and medication storage.
  • Performs patient call backs, medication counts, and observed urine drug screening in accordance with MAT policy and procedure as part of diversion control plan.
  • Ensures documentation of services provided are recorded in the electronic health record within 24 hours of provision, reflects the progress the MAT patient has made in response to the MAT services provided and is in sync with the details outlined on the MAT patient's care plan. Performs tasks such as opening and closing patient records necessary to maintain the accuracy of the electronic health records overseen by the MAT Program and documented in accordance with Sea Mar documentation policies and procedures.
  • Coordinates and collaborates with the MAT Provider and Medical Clinic staff working on site in support of the MAT Clinics occurring in the assigned region.
  • Quality Assurance: Data collection responsibilities including patient tracking and outcome measures required by grants and program evaluation.
  • Ensures that all medical, nursing, and MAT policies and procedures are being followed and implemented and reporting all deviations to the Program Director in writing immediately.
  • Order, track, store and administer injectable medication in accordance with Sea Mar policy reporting all deviations to the Program Director in writing immediately.
  • Other duties as assigned by MAT Program Leadership within the scope of employment and license.
  • Receives specialty training on contingency management and implements the intervention with study enrolled patient participants.
  • Manage the active patient census for the clinics involved in the study including proactive outreach, use of a clinical registry, and regular patient case reviews with program leadership staff an addiction specialist provided by the NIDA Clinical Trial Co-Care study.
  • Interfacing with Research Assistant and Research Coordinator to ensure timely collection and entry of research data elements.
  • Attend periodic and regularly scheduled Program meetings and research related trainings on topics relevant to the MAT Program, staff, the services provided, and the MAT patients served. Some trainings and meetings are unique to the associated study.
  • Other duties as assigned by MAT Program Leadership within the scope of employment and license.

Qualifications:
The Nurse Care Manager must posses the ability to solve practical problems and a variety of situations where only limited standardization exists. Must be able to make decisions independently and have a good sense of medical priority. Must be able to anticipate the needs of the patients and providers according to clinical presentation. Must be able to anticipate the needs of the clinic according to existing demands and resources and based on historical trends. Must be able to problem solve creatively in a variety of situations and circumstances. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Education, Certificates, Licenses, Registration and Medical Screening:
Nurse Care Manager must be a Registered Nurse licensed with the State of Washington, and have graduated from an accredited college or university with a BSN, Associates degree, or higher. The qualified applicant will understand patient engagement that aligns with Harm Reduction approaches and have experience working with individuals living with substance use disorder.

Training:
Must complete initial Training Plan consisting of assigned video and reading accompanied by discussion with Program Director and Program staff within the first 2 weeks of hire. Training plan content is directly associated with the assigned position tasks, Program philosophy and policies and general knowledge necessary for employment at Sea Mar. Must also complete basic overdose education and use of Naloxone. Additional training will be provided at the directive of the study and require mandated attendance.

Additional Requirements
  • Pre-hire and annual employee health screening required
  • Annual influenza vaccine required. Only exception is for employees with a medical or religious exemption approved by Administration. Employees with an approved medical or religious exemption must wear a mask at all time during the flu season.
  • TB testing annually required.
Condition of Employment:

The Nursing Manager must sign an oath of permanent confidentiality covering all patient related information and must receive a background check from the Washington State Patrol. The Nursing Manager must meet the organization's health screening requirements for tuberculosis screening and immunization status.

Language Skill:

Ability to read and write proficiently in English. The ability to speak and interpret in Spanish is ideal. The Nurse Care Manager must demonstrate clear and concise written and verbal communication.

Mathematical Skill:

Must be able to calculate medical dosages, i.e., mg. to ml. Should be able to analyze and present simple data as needed for reports.
