The Social Worker/Primary Therapist participates as a critical member of the team to provide highly personalized and therapeutic social services to, as well as be an advocate for, mentally disabled individuals.

Required Qualifications:


• Graduate of an accredited master school of social work and/or counseling


• Current licensure as an independent practitioner (LCSW/LISW)


• Experience in community social work


• Excellent customer service skills


• Self-motivated and accountable


• Highly organized


• Team oriented


Preferred Qualifications:


• Previous experience in an acute mental health or substance abuse facility


• Certified Case Manager (CCM)


Duties and Responsibilities:


• Completes biopsychosocial assessments


• Coordinates and leads group therapy


• Provides individualized patient care appropriate for the area of practice


• Understands and follows policy and procedure in addition to meeting licensure requirements


• Responsive to current safety and quality initiatives and sustains improvement


• Builds productive interpersonal relationships in every encounter


• Provide crisis intervention and/or short term counseling


• Links clients with community resources


• Maintains adequate working knowledge of local resources. Maintain up-to-date knowledge of entitlement programs, eligibility requirements and benefits for state and federal assistance programs.


• Actively participates in the department programming and treatment planning sessions


• Demonstrates teamwork


• Facilitate self-determination and self-care through the tenets of advocacy, shared decision making and education


• Coordinates family meetings during the discharge process for all patients to ensure safe and appropriate discharge


• Practice cultural competence, with awareness and respect for diversity


• Supervises social work students.

• Other duties as assigned


Physical Demands and Environmental Work Conditions:


• Must be able to see with corrective eyewear and hear clearly with assistance


• Must be able to walk, stand and sit for extended periods of time


• Must be able to lift greater than 20 pounds


• Fast-paced environment


Special Demands:


• Sets limits when dealing with angry, hostile, or sometimes verbally or physically abusive patients and families in an attempt to ensure a safe, respectful environment that will support the delivery of care


• Effectively copes and strives for balance when caring for acutely ill patients and families
