- Years of Experience REQ: 1 YR of long term care or related geriatric experience, 1 YR of experience working with the frail and elderly
- First-Timers Accepted: No
- Weekend REQ: Yes (occasionally)
- Certs REQ: BLS, ACLS
- Skills REQ: Experience with patients with Dementia, Alzheimer's, Substance Abuse. Hoyer Lifts, Sit to Stand. This is a full federal PACE program. The participants (patients) are surrounded by a care team here as well as adult day health, served lunch, and a skilled rehab gym. Their PCP, RN, PT, OT, RT, SW, Chaplain are all here to support them.

SPECIAL REQUIREMENTS
- The LPN / LVN participates, coordinates, and provides direct nursing care in a manner which ensures participant's satisfaction and retention. Assists with medication orders and delivery, coordinating with the physician, RN, transportation and home care. Assists physicians and nursing staff with a variety of tasks, including but not limited to vital signs, wound care, medication administration/management, treatments, patient teaching, venipuncture and microcoagulation, catheter care foot care, administration/reading PPD tests and participant set up for physician visit. Performs duties in a variety of settings including the community, the Center, and the Clinic. May be asked to perform duties on weekends.
- Candidate is expected to have reliable transportation and no additional scheduling restrictions. All travel pairs will be required to have their own transportation.
