Licensed Practical Nurse- Oswego Health Homecare

The Home Care LPN is a direct care provider to agency patients under the direction on an RN in the home setting.

We work under the direction of your physician, to provide services that can help to improve our patients' quality of life while offering convenience to their families and physicians. When under our care, patients are less likely to need urgent, unplanned medical attention. Oswego Health Home Care enables patients to live as safely and fully as possible at home with their loved ones.

Oswego Health Home Care is the only hospital-based certified home health care agency in Oswego County.

Current NYS Nursing License with a minimum of one year recent med/surg or home care experience required.

Pay Range: $24.00-$26.40/hr

Employee salary is based on commensurate experience and other qualifications.

The health system also operates Oswego Health Home Care, the only hospital-based certified home healthcare agency in the County as well as two outpatient centers, including the Fulton Medical Center, offering urgent care, lab, medical imaging, physical therapy, and occupational health services; and the Central Square Medical Center, offering urgent care, lab, medical imaging, and physical therapy services. In addition, Oswego Health includes the Oswego Health captive professional corporation, Physician Care P.C., providing physician services in orthopedics, cardiology, ENT, gastroenterology, breast care, audiology, general surgery, bariatrics, and primary care.
