Licensed Practical Nurse (LPN) Job Requirements: - Provide ongoing observation of individuals within the framework of holistic nursing practice
- Observe and report data regarding individual that may impact siccessful treatment outcomes.
- Identify and communicate individual's needs to the supervising RN, interdisciplinary health care team, and other providers as needed
- Provide ongoing monitoring of individual's response to interventions and report any deviations from expected outcomes to the supervising RN
- Coordinate and update an individualized plan of care based on evidence in collaboraiton with the RN, individual, their family, and the team to promote continuity of care.
- Indentify, initiate and coordinate therapeutic interventions consistent with the scope of practice and level of competence
- Implement prescribed interventions
- Consult with the RN and team to discuss any changes needed to achieve desired outcome
- Document complete, accurate, and relevant data for every individual assigned to their care for the shift as well as assigned primary individual caseload.
Candidate Profile: - Graduate of an approved Practical Nursing Program
- Current licensure in NYS as Licensed Practical Nurse
- Current Basic Life Support Certification
- 2+ years of work experience in nursing field
- Expereince working with individuals with intellectual or phsycial disabilities a plus
- OPWDD strongly preferred
- Expereince with SigmaCare and/or E-Clinical Works a plus
- Maintain required licensing and mandated training by the NYS Education Department
