Responsibilities â â â â

Implementation of physicians â â" prescribed plan of care â â â
Patient assessments and coordination of care â â â
Administration of prescribed medication, treatments, and therapies â â â
Compliant, timely documentation of care provided in system of record â â â
Health promotion, teaching, and training of family members â â â
Responsible for accepting and writing verbal orders from physicians as required and for obtaining co-signature of Case Manager on all verbal orders â
Follow up with physician, client, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition â â â â
Performs other appropriate duties as assigned â

Qualifications â â â

Must have and maintain an active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice â â â
Current CPR certification from American Red Cross, American Heart Association, or BLS â â â
TB skin test (current within last 12 months) â â â
Six months prior hands-on nursing experience preferred but not required â â â
Must have reliable transportation â â â

Qualifications of the LicensedPractical Nurse (LPN):

Must have good standing license (LPN) in the state in which the clinician will practice
Must have at least 6 months of hands on nursing experience
Current CPR certification
Must be willing to provide in home nursing care to infants, children, and adolescents

Responsibilities of the LicensedPractical Nurse (LPN):

One to one patient care in a home environment
Execution of physicians prescribed plan of care and compliant documentation of care provided in system of record
Administration of prescribed medication, treatments and therapies
Patient assessments, coordination of care
Health promotion, teaching, and training of family members
