Responsibilities: Provides services in accordance with nurse practice act, state/federal regulations and nursing community policies and procedures. Performs and documents resident evaluation on admission and on a continuing basis, as necessary. Monitors and documents changes in health status through continuing evaluation of the resident. Observes resident care needs, initiates nursing interventions and evaluates outcomes. Report resident physical and psychosocial status to nursing staff, physicians, resident's families and other departments, as appropriate. Transcribes and carries out physician's orders, as appropriate. Administers medications, as prescribed by a physician. Monitors resident for therapeutic results and side effects. Administers treatments and special procedures as prescribed by a physician. Completes all medical record documentation as required by accepted standards of nursing practice, federal and state guidelines. Assists physician as necessary during rounds and examination or treatment of residents. Actively participates in the care plan process. Assists residents with activities of daily living, as necessary. Assists resident with meeting their nutritional/hydration needs. Assists the residents in regards to regaining lost abilities and to maximize their strengths, to assist in creating an atmosphere of independence. Supervise and participate in resident programming. Maintain confidentiality of all resident's private health information. Offers emotional support to residents families, e.g., during admission adjustment, the grief process, coping with chronic illness and working with the cognitively impaired resident and his/her family. Acts as a team leader. Directs and supervises tasks of non-professional staff to assure quality of nursing care is maintained. Assists with the evaluation of staff. Initiates disciplinary action, as needed. Participates in community in-service programs by attending scheduled in-services, suggesting in-service topics, teaching other staff on an informal and formal basis, as required. Organizes work day according to accepted routines and prioritizes activities. Understands and implements the Fire Safety and Disaster Preparedness Plan and evacuation procedures of the community during drills and emergencies. Follow all safety, security, infection control, Universal Precautions and Hazardous Material policies and procedures. Performs all tasks to assure resident and personal safety and the protection of co-workers and community property. Interacts with residents, guests and staff in an atmosphere of hospitality. Helps ensure resident safety. Perform other duties as assigned Qualifications: To perform the job successfully, an individual must be able to perform each essential duty satisfactorily. Educational Requirements and Experience: Must be a graduate of an approved LPN program Must have, as a minimum, one (1) year of experience in a hospital, nursing care facility, or other related health care facility. Must possess a current, unencumbered, active license to practice as an LPN in West Virginia. Must be able read, write, speak and understand the English language. Basic Life Support Certification (CPR) or willingness to obtain Knowledge and Competencies: Must have basic knowledge of computers. Knowledge of nursing and medical practices and procedures, as well as laws and state/federal regulations concerning dementia care and skilled nursing care in West Virginia. Knowledge of the concepts of aging and concerns of the elderly. Conduct job duties in accordance with clinical standards of practice and standards. Possess the ability to make independent decisions when circumstances warrant. Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel. Organizes work to complete job tasks in a timely manner. Responds promptly to change in workload, adjusting assignments to achieve optimal productivity and efficiency. Verbal ability to express ideas and views effectively. Demonstrates attitude of cooperation and professionalism when working in community. Environmental Adaptability: Works primarily indoors in a climate controlled setting. May have exposure to unpleasant odors. Possible exposure to chemicals as identified in the MSDS Manual. Possible exposure to infections, diseases and blood borne pathogens. Continuous exposure to residents who are ill, confused, irrational and irritable. May have exposure to residents, family members and staff who are emotionally upset. May need to work beyond a normal working hour day, weekends and holidays, as necessary. Physical Requirements: Mobility is necessary in order to stand, sit, walk, use hands, fingers, stoop and crouch or kneel in order to interact with residents, staff and families at least 60 percent of the time. Talking, hearing, smelling, concentration, attention to detail, vision close and distance over 60 percent of the time. Lifting objects up to 10 pounds over 60 percent of the time and over 50 pounds less than 30 percent of the time.
