Hospice Licensed Practical Nurse
We are seeking for a compassionate Hospice Licensed Practical Nurse to provide care and support to terminally ill patients. Job Responsibilities:
- The Hospice LPN will provide compassionate nursing care and education to our patients and families under the supervision of the assigned RN Case Managers, and at times support the clinical team from the office.
- Complete appropriate documentation in timely manner
- Assist in auditing/compliance functions
- Assist with monitoring and ordering patient care supplies
- Inform the physician of changes in the patient's condition and needs, after consulting with a Registered Nurse
- Communicate with medical professionals, hospice team and related staff
Qualifications:
- Graduate of a Practical Nursing Program
- Valid LPN License in the state
- Minimum one year of experience
- Previous experience in hospice
- Willingness to serve on-call
- Must have reliable transportation & valid driver's license
