AuthoraCare Collective is currently seeking an LPN Homecare position for our Integrated Health Services (Skilled home-based-care) department supporting our High Point, Kernersville and Winston-Salem service areas. This position is Full-time: Monday - Friday 8:00am - 5:00pm. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area. Assume on call responsibilities as scheduled. Provides high quality nursing services under the direction of a Registered Nurse consistent with the agency's philosophy, policies and standards of care. Responsible for working within the scope of the Nurse Practice Act regarding judgment, nursing skills, teaching, and experience. Works with other members of the interdisciplinary team to meet the patient/family's goals of care. Work with physician to follow medical Plan of Care for the patient. Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching. Work closely with other members of the interdisciplinary group and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Assume on call responsibilities as scheduled. Education and Experience: Must be a graduate of an accredited LPN program. Must have a current North Carolina LPN license. One year's LPN experience is necessary, preferably two years in an acute care, hospice, LTC or home health setting. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area. Strong computer competencies needed including learning and using electronic medical record and documentation software. Ability to act with integrity, professionalism, and confidentiality. Prioritizing and good organizational skills. Patient management skills needed include excellent patient assessment and clinical nursing skills, critical thinking, problem solving, effective interpersonal and communication skills necessary. Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
