Lifesteps is hiring a Full-Time Community Homes Nurse for the Washington County area, that is based in Eighty Four, PA. Position Summary: As a leading role in healthcare provision for people with intellectual disability, the LPN nurse is key to supporting person-centered care and healthy outcomes. The Nurse will be will be a liaison and advocate for the IDD adult individuals in Lifesteps Community Homes. Nurses support and educate direct care staff and communicate health changes to supervisors, directors and medical professionals. The LPN may assist the individuals while on a medical appointment and obtain medical records and instructions from the doctor. The Nurse will verify medical recommendations are being implemented and staff is being trained in the care of the individuals' needs. Documentation is kept regarding client diagnosis and medications, and staff training. Microsoft Word & Excel is used during the clerical times of this position. Travel to an assigned area to check in on the needs of the individuals is required using a personal vehicle. Mileage reimbursement is available. This full-time position works a schedule of Monday through Friday 8 am to 4:30 pm with weekends off. QUALIFICATIONS: Required: Licensed Practical Nurse (LPN) or Registered Nurse (RN) with an active PA license. One (1) year experience working in a nursing field. Demonstrated ability to complete basic computer tasks. First Aid/CPR certification or the ability to complete course within 6 months of hire. Current driver's license and the ability to travel for business utilizing personal vehicle. Preferred: Experience with Intellectual Developmental Disability and/or special education. Experience in community nursing, home care and/or school nursing.
