SCOPE OF POSITION
The LPN is responsible for nursing care assigned to that position and directs others in patient care. Demonstrates the ability to deliver care based on patient needs appropriate to age and developmental level. Is responsible for the daily organization of clinical area, patient care, and other nursing team members with less experience. Should possess a basic working knowledge of nursing principles. Decision making limited by the organization's policy and procedures. This position with demonstrate competency in the care of patients ranging from infancy, childhood, adolescence, adulthood, and geriatrics.

It is vital that an individual in this position be capable of good communication skills. It is of the utmost importance that written communication is legible.

Job duties of this position may require access to protected patient information (PHI). The team member will be accountable for appropriate use of the record and compliance with all confidentiality and security policy and procedures related to use, access, and disclosure of PHI.

This position may also assist providers in documenting clinical interactions with patients, filling out paperwork, summaries and referral letters after a visit and entering data into medical records.

REPORTING RELATIONSHIP
The LPN reports to the Physician and the supervisor or administrator of the clinic. Must communicate with provider at all times.

EDUCATION AND EXPERIENCE
Graduate of approved school of nursing and currently licensed to practice in the appropriate state.

CONTINUING EDUCATIONAL REQUIREMENTS

a) Obtains 12 hours educational credit.
b) Verbalizes understanding of Shared Governance and willingly participates on committee, task force, or project to improve patient care or work area when asked.
c) Attends 75% of staff meetings and reads 25% of staff meetings missed.
