Summary The Observation Service (3 South) consists of two units working in tandem to provide care to ED and Medical Observation patients. Observation Service is a fast pace environment with a quick turnover of the patient population. The dedicated team includes providers, nurses, Department Coordinators, and multi-professionals that promote interdisciplinary collaboration. ED observation patients will stay from 8 to 24 hours, while Medical Observation patients will remain between 24 to 48 hours. Nurses on this unit will train to interpret telemetry and be stroke certified. There will be a variety of patient diagnoses and situations on the units at any given time. Delivering care that ensures patients return home promptly is at the core of nursing practice across Concord Hospital, especially in the Observation Unit, where interdisciplinary teams collaborate daily to move patients through the continuum of care seamlessly, swiftly, and safely. Shifts are 7 am - 7:30 pm or 7 pm - 7:30 am with rotating weekends and holiday obligations. Education High school or equivalent (GED). Completion of a certificate program for Licensed Nursing Assistant. Certification, Registration & Licensure Certification required: Current American Heart Association Basic Life Support for Healthcare Providers certification, or equivalent course credentialed by American Heart Association. Licensure required: Current New Hampshire State LNA license Experience Evidence of the ability to perform as a member of a team. Responsibilities Performs activities of daily living and personal care needs under the guidance and supervision of the Registered Nurse. Performs technical skills/procedures relating to physical, mental health and social needs of patients under the guidance and supervision of the RN. Documents care provided and reports to the RN observations of patient's physical, social and mental status and basic patient data collected. Demonstrates adherence to safe patient handling policy. Demonstrates time management skills. Maintains the patient care areas to ensure cleanliness, safety and supplies of material and equipment. Receives delegated work adhering to the five rights of delegation. Performs job specific requirements and other duties as assigned. Serves as a preceptor for new employees.
