Hospice/Home Health Licensed Nursing Assistants at the VNH are an integral part of the care team, where they provide personal care and other supportive services to individuals in their homes, assist the patient to live independently with dignity and safety, and provide temporary respite and relief from total care responsibilities for the primary caregiver .

The LNA will:
  • Under the supervision of the RN, Case Manager, perform patient care services according to the individual patient's treatment plan. Examples include: apply simple non-sterile dressings; assist with bowel and bladder programs; obtains vital signs; perform PT/INR testing, etc.
  • Assist patient in maintaining personal hygiene according to the individual patient's treatment plan.
  • Perform routine housekeeping tasks in order to promote a safe and comfortable environment for the patient such as changing bed linens, completing laundry/dishes, and preparing simple meals.
  • Observe and report to RN, Case Manager, any changes in patient's physical or mental condition. Complete documentation in a timely and accurate manner. Coordinate with other professional staff when there is a need to revise the care plan.
  • Accompany patient when they are permitted outside the home. Promote mental alertness of the patient through conversation and activity. Utilize infection control practices with the delivery of patient care services.

Minimum Qualifications:
  • Must be a Licensed Nursing Assistant in Vermont and New Hampshire.
  • High school graduate or equivalent
