With our residents in mind, we are seeking an experienced Social Worker for our Hillsborough location. This position is a temporary full time position for about 4-6 months starting in July.

Duties/Responsibilities:
Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each resident
Assess psycho/social well-being of patients.
Coordinate and participate in Care Plan Meetings
Acting as a liaison with social, health and community agencies
Discharge Planning
Develop relationships with physicians and families

Qualifications/ Skills:
Must be able to read, write and communicate in English
Experience in long term care preferred
Experience with screening, assessment, and planning for common social services needs
Strong interpersonal communication skills with exceptional active listening abilities
Highly empathetic, non-judgmental, and open-minded
Experience in a collaborative team environment

Education and Experience:
Certified or Licensed Social Worker in the State of NJ
Bachelors Degree (Required) in Social Work or Psychology. Master's Degree preferred
1+ years' experience in clinical social work
A problem-solving orientation and a flexible and positive attitude
