Hospice Care Social Worker Job Description:
Provide psychosocial education to patients and family caregivers about coping skills, hospice and palliative care philosophy, and nonpharmacological symptom management strategies
Facilitate psychoeducational support groups
Provide in-services to other service providers and organizations
Lead community education workshops
Plans for discharge, coordinates care, and helps clients navigate systems; identifying and linking clients with resources
Facilitates advance care planning and lifespan planning
Intervenes in crises
Mediates conflicts within families, clients, and the interdisciplinary team, and between service organizations
Participates in interdisciplinary team meetings, care planning, and ethics consultations
Advocates on behalf of the patient and family
Document social work activities

Requirements:

1-2 years Healthcare experience

BSW with one year experience or MSW with no experience.

Compensation

Benefits package include

$60,000 to $70,000 Depending on Experience
