Job Summary: The Licensed Practical Nurse (LPN) in a long-term care facility is a vital member of the healthcare team, responsible for delivering high-quality nursing care to residents. The LPN works under the supervision of the RN and collaborates with other healthcare professionals to ensure the well-being of residents while adhering to relevant regulations and standards.

Key Responsibilities:

Patient Care:

Perform nursing assessments and contribute to the development of care plans.

Administer medications and treatments as prescribed.

Monitor residents' health status and report changes to the RN Supervisor.

Supervision and Coordination:

Oversee and guide Certified Nursing Assistants (CNAs) and other nursing staff.

Collaborate with the interdisciplinary team to coordinate and provide comprehensive care.

Wound Care and Procedures:

Conduct wound care and other nursing procedures.

Monitor and document the progress of residents' conditions.

Documentation:

Maintain accurate and up-to-date documentation of patient care and assessments.

Ensure compliance with facility policies and regulatory requirements.

Medication Management:

Administer medications according to physician orders and facility protocols.

Monitor residents for potential side effects and drug interactions.

Communication:

Effectively communicate with residents, families, and other healthcare team members.

Provide education to residents and families about their conditions and care plans.

Infection Control:

Follow and enforce infection control protocols.

Educate staff on proper infection prevention measures.

Emergency Response:

Respond to emergencies and provide assistance during crisis situations.

Ensure the safety of residents and staff.

Quality Improvement:

Participate in quality improvement initiatives.

Contribute to data collection and analysis for continuous improvement in patient care.

Staff Training and Development:

Provide orientation and ongoing education for nursing staff.

Stay updated on industry developments and incorporate best practices into care delivery.

Qualifications:

Current and valid LPN license in the state of New Jersey

Graduation from an accredited practical nursing program.
