Job Status: Full and Part Time Positions

Pay: $25.72 - $29.91

1. Maintains and actively promotes effective communication with residents/patients/clients and family members / personal representatives.

2. Responsible for maintaining a positive image of the entity in the community keeping in alignment with our mission, vision, and values.

3. Prepares and implements plan of care.

4. Supervises duties of CNA's and assists with physician visits.

5. Administers and monitors medications and maintains a current knowledge base with minimal errors.

6. Observes, documents, and reports resident status including 24 hour report.

7. Notifies charge nurse and possibly family or physician of change in condition.

8. Assists residents to attain and maintain the highest practicable level socially and medically.

9. Performs medical treatments and reports progress or lack of progress to RN.

10. Signs physician orders after ensuring accuracy, appropriateness, and completion.

11. Provides adequate transfer of information to next team leader.

12. Maintains honest, open, professional communication with others as a positive role model.

13. Provides feedback to supervisor to assist in the evaluation of other staff.

14. Leads, guides, and directs CNA's in the provision of quality care.

15. Practices universal precautions and utilizes, appropriate personal protective equipment.

16. Performs other similar duties as needed.

Requirements

1. Commitment to quality programs and services for the residents/patients/clients.

2. Excellent communications and human relation skills.

3. Ability to relate well to the residents/patients/clients, family members / personal representatives.

4. Ability to maintain and protect the confidentiality of information.

5. Ability to exercise independent judgment and make sound decisions and exhibit flexibility when needed.

6. Mathematical ability to determine medication and fluid calculations.

7. Ability to speak, hear, and write legibly to communicate with residents, families, and staff.

EXPERIENCE AND TRAINING

1. Current LPN registration in the State of Wisconsin
