Connected-Mid Valley Home Health & Hospice is hiring a Licensed Practical Nurse (LPN) to serve in our new Willamette Valley area location (Salem/Woodburn). The LPN is responsible for following the patient's plan of care and providing treatment under the supervisor of the RN.

Essential Duties
  • Implement plan of care initiated by the registered nurse
  • Provide accurate and timely documentation consistent with the plan of care
  • Collect assessment data and performs observation to report to and collaborate with RN(s) and physician(s)
  • Participate in coordination of home health services with interdisciplinary team
  • Use equipment and supplies effectively and efficiently
  • Participate in personal and professional growth and development

Requirements and Qualifications
  • Valid Oregon Licensed Practical Nurse License
  • Maintain a current CPR certification
  • 2+ years of experience working in direct patient care
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
