The Hematology and Oncology Roving Licensed Practical Nurse is based out of the Alexandria Community Based Outreach Clinic (CBOC) in Alexandria, MN and will travel to the other CBOCs within the St Cloud Health Care System as required. The LPN is responsible to provide nursing care to patients according to established facility procedures adhering to all VA and nursing policies, procedures, standards, and protocol. Duties include but are not limited to: Serves on the Core Team for Oncology Care referring patients/spouses to appropriate resources. Collaborates with Oncology team members and prepare appropriate patients for procedures, bone marrow biopsy, transplant, treatments, radiation, chemotherapy, surgery, outside coordinated care. Communication Point of Contact (POC) between the PACTs and the Oncology service for all patients suspected of having a cancer diagnosis. Ensure patient is scheduled in a timely manner for imaging/labs/biopsies that are required. Monitoring patient through the care delivery needs as appropriate. Compliance with various Isolation Precautions and Infection Control Guidelines Travel to the CBOCs for St Cloud Health Care Systems required. Delivers safe, basic physical and emotional care to patient populations ranging from young adult to geriatric age groups, considering gender and culture. Performs medication administration and monitoring according to SCVAHCS policies and procedures. Participates in patient/family teaching programs related to health promotion and reinforces instructions provided by the registered nurse and physician and documents in approved format.
