Essential duties include: Provides nursing and supportive care in accordance with Interdisciplinary Plan of Care, under the direction of the RN. Communicates with the attending physicians, other home health team members and other agencies as needed to coordinate optimal care and use of community resources for the patient/family. Document all services provided and submit on appropriate forms. Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits. Report changes in patient condition and needs to RN Great time management skills Qualifications: Minimum of 1 year as an Licensed Practical Nurse (LPN) with Home Health/Hospice experienced preferred Current Arkansas license Participate in On-call with other LPNs' Have dependable transportation, drivers license and car insurance Exceptional People providing Exceptional Care.
