In order to make this possible, we need exceptional Licensed Practical Nurses who can meet the requirements below.

Summary of Scope:

The Licensed Practical Nurse (LPN) will be responsible for supporting the Team with effective Member care, partnering with clinical staff to deliver quality treatment and recovery services. Will monitor Members' health concerns, help with collaboration-of-care, provide appropriate screenings and referral to PCP, and provide education on wellness topics and self-care and collaborate with the Team to lead the Member community according to our Mission and Core Values.

Specific Responsibilities:
  • Provide health and wellness services for assigned members
  • Provide assistance per psychiatrist's directives with medication issues and monitoring
  • Provide appropriate screenings and referrals to PCP
  • Assist psychiatrist with members' collaboration-of-care
  • Provide groups, workshops, and engaging programming educating members on health and wellness topics
  • Teach members health management skills and support increasing independence in this area
  • Complete all documentation on day of service delivery
  • Assist with monitoring medication documentation for accuracy
  • Participate in case consultation as requested by Director or Clinicians
  • Conduct and document collaborative activity with various providers, referral sources, insurance companies, and regulatory agencies
  • Participate in inter-agency meetings when needed
  • Complete trainings as required for regulatory compliance
  • Participate in daily Morning Team Meetings and weekly Team Meeting
  • Participate in Annual Performance Evaluations

Required Performance Expectations
  • LPN will participate in a multidisciplinary treatment team and will support
  • and implement interventions and clinical directives as directed by the Psychiatrist the Clinician or the Team.
  • LPN will demonstrate in their work with Members at ALL times, compassion, and concern when supporting a Member through embracing our organization's Mission and Core Values.
  • LPN will approach Member engagement from a non-judgmental stance understanding that a Member's behavior is driven by past experience, which may include trauma.
  • LPN will treat and speak to Members with supportive kindness even when a Member demonstrates intense behavioral or emotional actions. Staff will show Members dignity and respect for their values and lifestyle.
  • LPN will seek out appropriate support, consultation with Clinician or Psychiatrist, in conjunction with the Program Director or obtain supervision when they are uncertain about how to respond or support a Member effectively.
  • LPN will report back to the treatment Team, Clinician, and Psychiatrist, any observations of Member behavior that suggests Member may need additional treatment interventions and/or support.
  • LPN will engage with all external parties/individuals with appropriate professionalism and with a positive customer service approach, understanding that they represent their employer at all times

Qualifications
  • A PA license from an accredited licensed practical nurses program.
  • A current and valid LPN license from State where job will be performed
  • A complete listing of all health care settings wherein applicant has worked as an LPN
  • 3 years of experience working with psychiatric population and forensic population preferred
  • Current CPR Certification
